The Hawaiʻi Administrative Rules, Chapter 11-157 requires all students to meet immunization and tuberculosis clearance requirements prior to the first day of classes. The Health Clearance form must be completed by a US licensed practitioner (physician, Advanced Practice Registered Nurse or Physician Assistant) or the Hawaiʻi Department of Health. All immunizations must meet minimum age and minimum intervals between doses.

All Students
You may not attend classes until you have received health clearances for the following:
- Tuberculosis Clearance (TB)
- Measles, Mumps, Rubella (MMR) - 2 doses
- Tetanus-Diphtheria-Pertussis (Tdap) - 1 dose
- Varicella - 2 doses
NOTES:
- Students who attended any UH Campus prior to Summer 2020 are only required to have the following: TB Clearance and MMR (2 doses).
Students Living On-Campus
In addition to the TB Clearance, MMR, Tdap and Varicella requirements, new students planning to live on-campus who are 21 years of age or younger are required to have completed at least one dose of the Meningococcal Conjugate (MCV) vaccine given on/after the age of 16.
Medical & Religious Exemptions
Valid exemptions for MMR, Varicella, MCV and TDAP immunizations include medical and/or religious reasons. No other exemptions are allowed by the State of Hawaiʻi. All medical and religious exemption documents must be uploaded through Med+Proctor.
Students Registered in Online-only or Distance Learning Programs
Students who are enrolled in fully online programs are exempt from submitting Health Clearances. However, if students wish to participate in any on-campus employment and/or activities, they must complete and submit all health clearances in order to do so.
Recommended Immunizations
- Serogroup B Meningococcal (Men B)
- Polio
- Human Papillomavirus
- Hepatitis A and B
Most of the immunizations listed above can be obtained at your local health provider’s office, state health department, pharmacy, urgent care center, or by appointment at University Health Services (www.hawaii.edu/shs/).
